7/21/14 on 4/9/13 the address was changed from 1814 N. APPLETON ROAD, Menasha to 5337 Grande Market Dr, Appleton (the admin site). On 7/21/14 after EMG spoke with the grantee, OPA agreed to change the address back (via on-line CR) because the grantee did not understand that it should be listing the service delivery sites in the 340B database, not the admin site 4/9/2013 Site reinstated.
